 The forms available on this page have been adapted from those produced by the Electoral Commission (you can read more about the Commission here) and are in .pdf format.

 You should download and print the form(s) you require, complete and sign personally and return to me at  the address on the form or to the address serving the area in which you are resident.

Voter registration Form For British, Commonwealth, Republic of Ireland or European Union citizens to register to vote.
Application for EU citizen to vote in European elections EU citizens may choose to register to vote in relation to European parliamentary elections in the UK rather than their home state (and see below for other languages).
Application to vote by post If you will be unable to vote at your allocated polling station you can choose to vote by post.

If postal voting is not convenient eg you may be away from home on holiday etc close to the election or for other reasons you can have someone else (a proxy) vote on your behalf.
Application to vote by proxy at a single election If postal voting is not suitable or convenient you can have a proxy vote for you at a particular election.
Application to vote by proxy due to disability If you require someone else to vote for you at all elections, permanently or for a fixed period.
Application to vote by proxy due to employment If due to the nature of your or your spouse or civil partner’s employment you cannot vote in person, you can appoint a proxy.
Application to vote by proxy due to educational course If you are away from home because you are (or your spouse or partner is) attending an educational course you can appoint a proxy.  
Emergency application to vote by proxy In the case of a medical emergency taking place 6 days or less before an election you can apply up until 5.00pm on election day.

If you are a British citizen living overseas you are still able to vote at certain elections in the UK provided you were registered to vote in the UK within the last 15 years.
Application to vote as an overseas elector This form also includes applications to vote by post or by proxy depending on your circumstances.

Members of the armed forces (and spouses or civil partners) posted away from home can register as a “service voter” by completing this form.
Application to register as a service voter This form also includes applications to vote by post or by proxy depending on your circumstances.

Crown Servants or employees of the British Council (and spouses or civil partners) who are working outside the UK can register to vote in UK elections
Application to register as a Crown Servant/BC employee This form also includes applications to vote by post or by proxy depending on your circumstances.
